There are two parking lots located on each end of the shopping center. My office is above Black Sheep Jui Jitsu, across the street from Good Earth. If you walk behind Carmen’s Hacienda, the staircase is to the right of the nail salon. Take a left when you get to the top of the stairs and 735B will be on your left!
Do you take my insurance?
No. I am an out-of-network provider. I can provide you with a superbill to submit to your insurance (for all health insurance companies except Medicare and Medicaid), but can’t guarantee reimbursement. That will depend on your out-of-network benefits.
What is your cancellation policy?
I would appreciate 48-hours notice if you need to cancel so that I can open your appointment time to other patients. If you cancel with less than 48 hours notice, you will be charged the full cost of your visit.
Do you accept FSA or HSA?
Yes! You can pay with the card given to you or I can send you a SuperBill.
