Do you take my insurance?

No. I am an out-of-network provider. I can provide you with a superbill to submit to your insurance (for all health insurance companies except Medicare and Medicaid), but can’t guarantee reimbursement. That will depend on your out-of-network benefits.

What about Medicare?

Activate Physical Therapy is not a Medicare provider.
Because of this:

  • Medicare will not cover services provided here.

  • Supplemental insurance will not pay for services that Medicare does not cover.

  • Claims cannot be submitted to Medicare or supplemental plans for reimbursement.

You are welcome to schedule wellness or performance sessions on a self-pay basis. These visits are not eligible for reimbursement through Medicare or insurance.

If you have Medicare and choose to proceed with cash pay, it is imperative to print and sign an Advance Beneficiary Notice (ABN) before your first appointment.

What is your cancellation policy?

I would appreciate 48-hours notice if you need to cancel so that I can open your appointment time to other patients. If you cancel with less than 48 hours notice, you will be charged the full cost of your visit.

Do you accept FSA or HSA?

Yes! You can pay with the card given to you or I can send you a SuperBill.
